Abstract
Gender identity issues are increasingly dividing society, experts and those affected. There is talk of ongoing discrimination, the reversal of progress that has already been made or demands for it to be reversed, as well as the rejection and ostracism of binary positions. In reality, there have been significant changes in recent years: the massive increase in the number of people defining themselves as transgender in numerous countries with a simultaneous reversal of the sex ratio, the change in the law towards “self-determination of gender” in Germany and the controversy surrounding hormonal influence on puberty in adolescents. A differentiation is proposed with regard to the concept of gender (sexuality), with regard to the distinction between biological sex (sex) and forms of gender role and identity (gender) and with regard to the differences between “transsexualism” (ICD-10) or “gender dysphoria” (DSM-5) and other constellations affected by the “self-determination law”.
